What can cause a gallon of diesel fuel to show up in the crankcase oil? Is it possible for an injector to leak this much fuel passed the rings, start and run with out a puff of smoke?

usually the injector will not do this. To do this it would have to do it while it was running. The engine would have to be smoking horribly and for quite some time of running. Have seen engines running terribly from a rocker arm broke or some other ailment that will cause unburned fuel to get in the crank case. Never on a 6.2 or 6.5.
is there the slim to non possibility that the injector stuck open and the fuel pump just pumped it in there?

I remember a friend had an old 6.2 that the pump failed somehow and it leaked diesel into the crankcase via the timing and gear area. It was caught quick enough and the pump was repaired with no other problems.
I've seen it happen on a 6.5 with a couple cyls not fireing from a loose rocker shaft assembly. The valves wouldn't run on 2 cyls so the fuel had no way to burn and leave. The only place it could go was into the crankcase. It ran terribly rough and only added about 1/2 gal to the oil after run for an hour or so.
ok, what about if the front seal on the pump blew out, thereby sending diesel into the crank case via the timing cover entry, as previously mentioned?
"Technically" the 6.2 and the 6.5 pumps can't leak fuel into the engine because they incorporate a weep hole in the bottom of the housing. Seal fails, fuel will be leaking into the engine valley under the intake.
Well, if the weep hole didn't weep, there could become a bladder infection I guess. Or gallstones or in this case galled bearings. I ain't no doctor.
