Actually looks like a slotted shackle head pin, pretty common for boats, hex shackles too. That and it is pretty hard to find a carriage bolt not completely threaded

I bet it's not even a grade 8 carriage bolt.Nobody even noticed that its not a shackle pin there but instead he is using a carriage bolt.

And none of you professional tow men fell to see that cotter is a bump away from sliding out just from the weight of the hanging safety chain...last but not least do you guys really think that cotter pin is NOT going to pull out if the trailer were to break away from the receiver..
If anything the driver should have put a shackle d ring in that hole to the left of the hitch and attach his chains to the shackle d ring
