I have been reading all the threads on the injector failures on the LB7's however , no one has ever listed a concrete cause for these failures.
With the GM warrentee extention I am assuming a design issue was found
with the current OEM injectors.
Can one of the GM techs better explain the cause of these failures.
I love this Duramax GMC truck and would like to have a better understaning for the issues on the LB7's.

It's my understanding there were two issues. #1, the "O" ring seals in the injecter tip were inadequate. #2, the original stock fuel filter was not designed properly, GM redesigned the filter to a 2 micron filter media froma 10 micron. The filter was allowing too much dirt into the high-pressure common-rail system. The re-designed filter has a black band around the top half. This was somewhat like a wet sandblaster to the injecter seals. I'm just repeating what I was told by a Chevy service manager.
Changing filters often seems to greatly reduce the odds of injecter failures.

There was also an issue with the early injectors (used in the '01's and '02's) with injectors actually devealoping cracks, which's been fixed in subsequent versions of the LB7 injector.

Was there ever a recall by GM to replace the stock filter? Seems I heard something about this.

It was not a recall. However, they had a program that offered a free filter change at the dealer in an effort to get everyone running on the new filter . I think the program ended September 2006.

Correct. I'm not sure when it ended, but there was an open recall put on every Duramax truck for one fuel filter replacement while they phased in the new dual-pleat filters...
