Do you have a rear transmission sensor?

I'm trying to figure out how various trucks came from the factory so I can include 4X4 automatic trucks in the next DB2 Conversion guide and I need some input from 4L80E 4WD truck owners. Can you tell me:

1 - Your truck configuration. Example: 1995 2500 crew cab

2 - Type transfer case (if known)

3 - If you have a rear transmission sensor (see pic) and a transfer case speed sensor or only a transfer case speed sensor and a cover plate where the trans sensor would go. Please answer the poll as well.

My 98 2500 HD(8 bolt pattern) e-cab long box has two sensors but only 1 (closest to engine) is being used

on my 95 2500 LD(6 bolt pattern) e-cab short box it has a plug on the rear sensor. both trucks have the floor shift T/C
