Rattling noise when pulling

My engine seems to 'rattle' when pulling at 2500 rpm. If I back off from 2500 it stops. Recently it seems to make the noise only quieter at low speeds even when not pulling - just push on accelerator a bit.
Is this normal? And what is causing it?

EDIT: It doesn't happen all the times when at low speeds - just sometimes.

It's probably a harmonic noise from something loose in the exhaust; loose clamp or bolt somewhere. See if you can make it happen not under a load, parked. If you can, it's probably exhaust related, if not it could be a U-joint.
