01 dmax, has white smoke blowing at a good rate out of the breather. If you block the pipe it builds up pretty good. Threw a p0093 code once when i was driving it back to shop up a hill. Has not thrown a code since even with all the testing. We did injector balance test, #4 is reading +10. When we kill #4, the slight knocking goes away. Not really smoking through tail pipe, but does smell like bad injectors. There was fuel in crankcase that was drained when we noticed the smoking. After putting new oil in and running the tests, The new oil is showing a grayish color.
I am thinking we have a bad injector AND possible leaking injector cups...
Any info would be great...can't find anybody w/ the same multiple problems on any forums...

It sounds like you've got the injector figured out. As far as blowby, the crankcase could be overfull if you are getting alot of fuel in it from bad injectors. It is hard to see on the dipstick because the oild is so thin with fuel in it. Several ways to check for a head gasket / injector cup leak, but there is no way to tell them apart until you start tearing it apart. One thing is to pull glowplugs and pressurize the coolant system to see if any cylinders get coolant in them, also check for tiny bubbles in the coolant reservoir while the truck is running, discoloration of the coolant, have any oil analysis done by Blackstone to see if you have coolant in the oil. 01's are prone to head gasket issues and I would not suspect an injector cup to leak unless injectors were recently replaced.

well if the injector is dumping alot of fuel, and u think its mixing with ur oil, then when you go to check ur oil level, if oil is reading higher on the dip stick than what it should be, then there is some type of other liquid being put into the oil. coolant or oil. are you losing coolant with no signs of it leaking external?

We already tore it down to do the H/G's because of coolant blowing out the overflow when towing. Every gasket, o-ring, hose,anything rubber was replaced w/ factory parts.
Heads were sent to a machine shop to get decked and a valve job. They had all the new o-rings to install. Did not replace injectors because they truck ran fine and was not smoking ( this is my 4th complete set of injectors - 209,000mi on truck) When truck was back up and running, it ran way quieter and smoother than it ever has. No leaks. Just after the 2 days of driving the smoking started. I was hoping when we killed #4 that the blow by would stop, but it is puffing out of there like it was an exhaust pipe. As for the oil, when we drained before the testing it was black and thin-smelled of fuel. i filled it up a little short on purpose to check the level. Oil is now the grayish color.

My truck smoked like a bastard after injectors were done turned out I had a broken injector hold down.

Ya it was pouring out the breather tube, two had broke within 24 hours of being put back together.

If you just did head gaskets and now you have gray oil my vote is the injector cups aren't sealed properly, otherwise I'd say you're looking at redoing the head gaskets. Did you seal the cups or did the machine shop? I just got done doing head gaskets on my 04, I brought the heads to a machine shop but I resealed the cups myself. They do make a tool to remove the cups while they are still in the heads. My guess is if one is leaking it will pop out when you pull injectors.

Injectors were sent in the head to the machine shop. We took the new seals over to them when they got to that point of re-install. They are a pretty big outfit over there... We did pull that glow plug yesterday on #4 and it does have the grayish oil on it and smells the same burn coming from the breater hose.
