Rough idle, well if it's happening in gear say with your foot on the brake pedal it could be your fuel pressure regulator. If your not already, try running some type of fuel additive Ie Power service, stanadyne and see if that clears it up. You might also wanna take a look at the schrader vavle to, make sure it's not leaking, it's located near the alternator and it has a black cap on it.

I sometimes get that same problem, almost sounds like I have a cam on it. Have not figured it out yet.
