add injectors to that list
OMG, man everbody goes straight to injectors. Have you ever lost prime? Like when the fuel gets low? MANY have had similar issues including me and it was a SUPER SIMPLE o-ring, under the fuel prime button.
Lets do the cheapest/easiest stuff first.
The way to test if you need a new one of these o-rings, is to pump the prime button when you suspect you might have this difficult start issue, then see if it starts easier.
Also a sure fire way to know if you need it, is to pump the crap out of the fuel prime button and see if you see any fuel leaking on top of the fuel filter body under that stainless triangle retainer at the base of the button. If so PM me I have all the o-rings that leak and can slip them in an envelope and send them to you.
If its none of that, then more details about how it gets difficult, would be great. Like does it turn over slow, or is it just that it starts then dies, a few more details would be great.

Because 7 out of 10 times thats what it is.
Start with filters, check the o-rings, gaskets etc. If those check out, try pressurizing the tank to check for small leaks in the fuel system. These could cause some issues. Next I would look at any of the low pressure rubber fuel lines under the hood. If you dont find anything unusual in those areas, it could be a cracked filter housing. Rebuilt ones are offered, cant remember a price off the top of my head though. Also a bad fuel pressure regulator could cause a problem, but is usually accomponied by a rough idle. Past that, maybe a weak CP3 or finally injectors.
Have you checked the truck for codes? That could be a big help too.

Check the simple stuff first, i agree with that, but hard or no warm starts is a sign of injector issues.
acpinter 09-13-2009, 07:19 PM OK, so i changed my fuel filter and it helped out alot. I've got a friend who has one and his will fire right up. My truck cranks over fast but takes a few seconds to finally kick over. When it starts it stays running and runs pretty smooth.
My truck was acting up about 4 months ago by doing the same stuff but it would fire up and die after a few seconds. Then I would try to start it again and it would crank and crank until it stopped cranking, then I would do that for 5 to 10 minutes and it would finally start. It would run fine after that until I would shut it off and sometimes it would fire ok and others it would act up the same way. One of the times while trying to get it to start it threw a code that the crankshaft position sensor was bad. It only threw that code that single time. So ended up changing the fuel filter and seemed to fix it but it still cranks a few seconds till it fires up.

Not to rain on your parade, but changing the filter also got rid of my problem, but only for a few hundred miles. Each time I changed it, it happened sooner until constant no starts.
