There's several other posts about this on the boards too that I ran across when I started having the problem. Several people mentioned running some 2 cycle oil through a tank or two of fuel.
I gave that a shot and ran 2 qts in one tank, and 1 qt in a second tank, and it cleared it up for me as it did for many of them.

I am asking everyone to try this, because I have the same kind of problem.

When the problem goes away or is not happening, put(keep) your foot on the fuel just enough to not even bump the RPMs. It should to idle with the problem and mine smokes as well while doing this.

I also know my #7 is out of spec and is going in next week to be looked at. There are also some posts that talk about this problem going away after new injectors. It could be hand and hand.

Please let me know if you truck does it will you on the peddle.

I put 6 ounces of Opti-Lube in a tank and the idle greatly improved. It is now starting to make a come back.

This idle issue can also be flew while just idling in gear until you put your foot on the brake.
