Thinking of putting on a MBRP 4" T-409 single cat-back on my 04 GMC, CC, 4WD SB... I've got a spare tire with a 285/75/16 on it... any clearnace problems???

If you can get 2" of clearance, that should be enough, not too hot. I have heard that the aftermarket exhaust leave little if any clearance for larger spares. I have a 315/70/17, a 34.5" tire, about the same as your 285/75, and I have 2" to the stock exhaust. So I would guess you'll be close to touching. How much clearance do you have to the diff cover? I have 1/2", with the spare touching the hitch reciever.
I noticed you can move the spare around a little while hoisting it up, that might be the ticket.

CMC-GMC,
On a 285 you shouldn't have a problem at all, once you get into the 315's or more it gets real tight but guys have made it work even with then in the past. Give us a call if you need any help or have any questions, we have been selling the MBRP systems longer than anyone in the good ole US of A.

Is there a 5" exhaust that will clear the H2 spare?

Don't think so.
