My Dmax has a tough time starting after it is warm. No problem when it's cold. Just had the fuel filter changed last week, still showed same characteristics. I have had luck giving it gas while trying to start, but it seems to be getting worse. When it is running, it seems to "cycle" a bit, not the steady diesel sound we are all used to. I have 102K miles but not blowing any smoke and getting 18 MPG. Any ideas??

Could definately be injectors. My dads truck is doing the same thign and we were talking to one of our friends thats a Dmax Mech. and he said more than likely injectors.
what about the fuel pressure regulator?

O yea... Take the truck to the dealer and let them diagnose it.

Definitely injectors. You have one or more with high return rates, bad balance rates. If they are not beyond the GM limits, they will not authorize the dealer to replace them under the special policy. It will get worse.
I'm betting it even takes more than 2 seconds to fire when cold, maybe 4 or more? And changing the filter makes it better, for a while. See my posts. This is exactly what my symptoms were. No smoke, and no fuel in the crank case either.
