Has anybody actually broke the two crank pins? If so does anybody have pics?

Are you talking about the ones that locate the damper on the front of the crank?? If so, yes, I've done it.

Now the hard part is determining which came first: the chicken or the egg, AND whether or not the balancer was ever removed for service and/or torqued properly?

It is a "known" OEM problem, not to say it's common, but it has happened on many trucks. I purchased a used LB7 engine with sheared pins, and I have seen another truck that was idling so rough I thought the engine was going to break loose.

These engines are HEAVILY externally balanced, and if that dampener rotates it is a serious problem.

Yes, I thought the motor was going to shake itself to death. Four to six inches of movement at the extreme. And not only at idle. Very concerning, till problem diagnosed.

Repair came close to requiring a new crank, as dealer took over a day to remove the remnents of the sheared pin from the crank.

Easy in my case; egg. Sheared right after dealer replaced waterpump, which entailed removing the balancer.
