I'm thinking of running my cb cable through the vent, I have heard that there is foam in there. But my real question is do you drill the hole from the inside ou tor what. and should i take the piece inside off first or just go through it where it is? I probably made this sound more difficult then it really is. Thanks for any advice.

I have heard of people running teh cable in there but then you have to run it under the carpet all the way from the back of the cab. This means removing the seats (I think)
I added a rearview camera and used the e-brake hole in front of the drivers seat. Once through there it's easier to fish the cable up to the center console.
Here is a pic from under:

And inside in front of drivers seat:

The firewall on the drivers side to the left of the brake fluid reservoir (as you look under the hood) has either 1) a grommet w/wire going thru the firewall probably 5/8" or larger in diameter or 2) a flat, round indentation where this can be drilled for this purpose. My truck did not have the grommet. I went and bought a grommet and drilled a hole to fit it. that is where I have run my cb ant, hi idle, and winch solenoid wires. If you search the forum for firewall or hi idle you may be able to find some pics.
good luck
